How to request a copy of your records
(Cómo solicitar una copia de sus registros médicos)

Your records are managed and kept secure by ADC’s Health Information Management Office.

You can request a copy of your medical records by completing a release form. The form must be filled out completely, and it must be signed and dated by the patient or patient’s legal guardian (legal proof is required).

Where do you send your completed form?

Forms cannot be submitted by email or through unsecured electronic means. (Learn why.)  Send your completed forms by mail or fax to:

To pick up copies of records in person, please make arrangements in advance. You may call our office at 512-901-4153.

Photo ID Required

If you choose to send someone to pick up your records for you, we require a written letter of permission from you, specifying that person’s name. The person will be asked to show identification at the time of pickup.

How long does it take?

ADC has 15 business days once a properly completed release form is received.

Fees:  You will receive an invoice from CIOX Health, our professional copier service.

Material and Service Fees for Patient requested records (does not include postage):

  • Electronic medical records delivered in electronic format = $6.50
  • Electronic medical records and paper records delivered in electronic format = $6.50 plus $0.07 per page
  • Electronic medical records delivered in paper format = $0.90 plus $0.05 per page
  •  Paper records delivered in paper format = $0.12 per page.
  • Total fee NOT to exceed $25.00

Retrieving your Medical Record

There are several ways to retrieve your medical records:

  • You (or a designee – see above under “Photo ID Required”) may pick up the records from the ADC Medical Records (HIM) department.
    • ADC North (inside St. David’s North Austin Medical Center)

  • CIOX Health can mail the records to your home
  • Records can be placed in CIOX Health Portal for the you to retrieve.  To use the CIOX Heath portal please specify that you would like a digital copy and included your email address.  CIOX Health will send you an email when records are ready with instructions on how to retrieve the records from their secure portal.

About electronic medical records (EMR)

We’ve been using and developing our electronic medical record system since 2000, and as technology changes, we continue to update and improve. The system allows your ADC doctors to access your lab results, images or other information instantly.

We encourage you to access your health information through MyADC, our patient portal.
