How to request a copy of your records

Your records are managed and kept secure by ADC’s Health Information Management Office.

You can request a copy of your medical records by completing a release form. The form must be filled out completely, and it must be signed and dated by the patient or patient’s legal guardian (legal proof is required).

Where to send your form

Forms cannot be submitted electronically.  Send your completed forms by mail or fax to:

To pick up copies of records in person, please make arrangements in advance. You may call our office at 512-901-4153.

Photo ID required

If you choose to send someone to pick up your records for you, we require a written letter of permission from you, specifying that person’s name. The person will be asked to show identification at the time of pickup.

How long it takes

ADC has 15 business days once a properly completed release form is received.

Fees

Personal copies

  • 00-15 pages no charge.
  • 16-20 pages $1.00 per page, plus $0.50 for pages 21-40 plus postage and shipping.
  • 40+ pages $15.00 maximum plus postage and shipping

Transfer Records

  • 00-15 pages no charge
  • 16-20 pages $1.00 per page, plus $0.50 for pages 21-40 plus postage and shipping.
  • 40+ pages $15.00 maximum plus postage and shipping

You will receive an invoice from CIOX Health, our professional copier service.

About electronic medical records (EMR)

We’ve been using and developing our electronic medical record system since 2000, and as technology changes, we continue to update and improve. The system allows your ADC doctors to access your lab results, images or other information instantly.

We encourage you to access your health information through MyADC, our patient portal.
