We welcome your comments and suggestions.  This Feedback Form is forwarded to the Marketing/Public Relations Department.   If you wish to alert us about a problem with one of our Clinics please include the physician, staff member, and/or department in the body of the email.

While we welcome your feedback, it is important that you realize communicating with ADC via this feedback form is not secure.  Please limit your comments or questions to administrative tasks, ie: appointment issues, billing questions, seminar registration, comments regarding this website, comments regarding our business operations, etc.  You should assume that information will be forwarded internally.  This form should not be used for emergency matters or sensitive and confidential  medical information.  Failure to adhere to this practice may lead to confidential medical information being disclosed to parties, which have not been authorized by you to receive such information.
Please do not send questions or comments related to, or containing information about, your medical condition to the Webmaster.  This is not a secure site.
