Diabetes Assessment: Have
your blood sugar, cholesterol, blood pressure, and kidney function
tested routinely
Diabetes Target Attainment: Reach target goals set by the
American Diabetes Association for your blood sugar, cholesterol,
blood pressure, and kidney function
Diabetes Complication Prevention: Avoid complications from
diabetes by smoking cessation and being current on your flu
vaccination, pneumococcus vaccination, eye exams, and foot exams
