Where is Medical Records (Health Information Management
aka HIM) Located? We are located inside St. David's North Austin Medical
Center on the Ground Floor (basement) of the North Wing.
What are our office hours? 8:00A.M.– 5:00P.M., Monday through Friday.
How do I request a copy of my Medical Records? You can request a copy of your medical records by
completing an Authorization to Release Protected Health
Information. The form must be completed entirely and must be
signed and dated by the patient or patient's legal guardian
(legal documentation/proof required). At this time the form
cannot be electronically submitted.
Once completed the form may be mailed to, or dropped off
at the : ADC Release of Information Section.

Requests from patients only may be faxed to 901-4126. All
other requestors (insurance/attorney/etc.) must be mailed.
To pick up copies of records in person, please make
arrangements in advance by calling the ROI Desk. We require
photo identification at the time. If you choose to send
someone to pick up the records for you, a written letter of
permission from you is required and must specify that
person's name. The person will be asked to show
identification at the time of pick up.
How long does it take to receive copies of medical
records? By law, ADC has 15 business days once a properly
completed request including payment is received. If a
payment is required an invoice will be mailed to you within
10 business days of receiving the original request, and the
records will be copied only after payment in full is
received.
Is there a fee for releasing records? There is a minimum $15.00 fee for personal copies or to
transfer records to another health care provider that must
be paid in advance. Please make check payable to Healthport
which is ADC's release of information service. If your
request is more than 100 pages in length, an additional per
page fee of $.50 per page will be required. You will be sent
an invoice from Healthport. There is no charge for a copy of
one visit/test result.
If prepayment is not received, you will be sent an invoice
from Healthport. Once payment is received the records will
be mailed out.
