The Next Generation of Prostate Cancer Surgery

Surgical removal of the prostate and surrounding tissue (radical prostatectomy) has long been recognized as one of the most effective forms of therapy for prostate cancer. A conventional radical prostatectomy involves an open incision across the lower abdomen. Laparoscopic prostatectomy represents a major advance in the treatment of prostate cancer by offering men a less-invasive alternative to traditional surgery.

da Vinciâ Prostatectomy (dVP)  The da Vinciâ prostatectomy (dVP) is a minimally invasive surgical technique that provides a safe and alternative way to remove the prostate gland in patients diagnosed with prostate cancer.

The operation is performed using the da VinciâSurgical System and fine laparoscopic instrumentation inserted through 5 to 6 small 1-cm incisions across the mid abdomen. This is in contrast to the conventional open radical prostatectomy where a lower midline abdominal incision is required for dissection and removal of the prostate gland.

During the dVP, a telescopic lens is inserted into one of the small 1-cm incisions, providing a magnified view of delicate structures surrounding the prostate gland (e.g. nerves, blood vessels, muscles, and bowel) thus allowing optimal preservation of these vital structures. The cancerous prostate gland is dissected free from the bladder and urethra, and the bladder and urethra are sewn together without the surgeon's hands ever entering into the patient's body. The prostate is eventually removed intact through one of the small 1-cm incisions located at the belly button by extending the incision to accommodate the prostate depending on its size (usually 3-4 cm). 
 

The da VinciâSurgical System

The da Vinciâ Surgical System is a computer-enhanced minimally invasive surgical system consisting of three components: the InSite Vision System, Surgical Cart and Surgeon Console. The InSite Vision System provides the surgeon a true minimally invasive 3-D view of the surgical field including depth of field, magnification and high resolution. The Surgical Cart includes three robotic
arms whereby the laparoscope and two EndoWrist Instruments are inserted. The EndoWrist Instruments are designed to mimic the movement of the human hands, wrists and fingers. The extensive range of motion allows precision that is not available in standard minimally invasive procedures. The Surgeon Console contains the master controls that the surgeon uses to manipulate the EndoWrist instruments. The handles or “Masters" translate the surgeon's natural hand and wrist movements into corresponding, precise and scaled movements. The EndoWrist Instruments are only able to move when commanded by the surgeon.
